The Footy Lab
Participant Waiver, Release of Liability, Assumption of Risk, and Media Consent
By registering for and participating in any event hosted by The Footy Lab (including but not limited to training sessions, clinics, and camps), the undersigned parent/guardian of the participant (the “Player”) acknowledges and agrees as follows:
1. Assumption of Risk & Release of Liability

Participation in athletic activities carries inherent and unavoidable risks of injury, illness, permanent disability, or death.

Such risks include, but are not limited to:
• Collisions with other players, equipment, or surfaces.
• Falls, slips, or contact with the ground or goalposts.
• Overexertion, muscle strain, dehydration, or heat-related illness.
• Sudden cardiac arrest or undiagnosed medical conditions.
• Adverse or extreme weather (rain, heat, wind, lightning, smoke, poor air quality).
• Unsafe, changing, or defective field/facility conditions.
• Acts of nature, accidents, or other unforeseen circumstances.
• Transportation to, from, or during the event.
By registering, I knowingly and voluntarily assume all risks, whether known or unknown, foreseen or unforeseen, even if arising from the negligence of The Footy Lab, its directors, officers, employees, contractors, volunteers, or facility partners (collectively, the “Released Parties”).
I hereby release, discharge, and covenant not to sue the Released Parties from any and all claims, liabilities, damages, or expenses (including attorneys’ fees) related to participation, including those arising from ordinary negligence, except where prohibited by law or caused by gross negligence or willful misconduct.
2. Medical Authorization

I certify that the Player is in good health and physically fit to participate in soccer and related athletic activities.

I agree to provide written notice of any medical conditions, allergies, or medications prior to the event.

In the event of illness, injury, or medical emergency, I authorize the Released Parties to secure medical care for the Player, including transportation, hospitalization, surgery, medication, or other treatment deemed necessary by qualified personnel.

I assume full financial responsibility for all medical expenses incurred on behalf of the Player.
3. Media Release

I grant The Footy Lab the irrevocable right to photograph, film, record, or otherwise capture the Player’s likeness, image, voice, and statements during the event.

I authorize The Footy Lab to use such media for marketing, educational, and promotional purposes in any format (print, digital, broadcast, social media, advertising), worldwide, in perpetuity, without compensation.

I waive any right to inspect or approve materials and release all claims related to ownership or use of the media.

Opt-Out: Parents/guardians wishing to exclude their child from media use must email cody@thefootylab.com at least 7 days before the event and notify staff at check-in.
4. Refund & Transfer Policy

All registrations are final and non-refundable.

Registrations may be transferred to another eligible player at no cost until the event start time.

Refunds will only be issued if The Footy Lab cancels the event in its entirety and no reasonable reschedule option exists.

No refunds or credits will be provided for:
• Late arrival or early departure.
• Player illness, injury, or scheduling conflicts.
• Weather delays, unsafe conditions, or other unexpected events beyond our control.
In exceptional circumstances (e.g., verified medical emergencies), The Footy Lab may, at its sole discretion, approve a partial credit toward a future camp. Such credits are not guaranteed.
5. Code of Conduct
Participants and parents/guardians agree to:

Follow staff instructions at all times.

Treat staff, coaches, players, and facilities with respect.

Avoid unsafe, disruptive, or unsportsmanlike behavior.
The Footy Lab reserves the right to remove any participant without refund if behavior endangers others or violates these policies.
6. Weather, Facilities, & Unexpected Conditions

Sessions will proceed in rain when safe to do so.

The Footy Lab reserves the right to delay, shorten, relocate, or reschedule sessions in case of:
• Unsafe weather (lightning, excessive heat, wildfire smoke, poor air quality).
• Facility closures, unsafe field conditions, or equipment failures.
• Public health restrictions or emergencies.
• Any other unexpected or extraordinary conditions that, in the sole judgment of The Footy Lab, pose a safety risk.
If no reasonable reschedule option exists, refunds will be issued only in accordance with Section 4 above.
7. Parent/Guardian Acknowledgment
By completing registration and payment, I certify that:

I have read and understood this Waiver, Release, and Policies.

I acknowledge I am waiving substantial legal rights on behalf of myself and the Player.

I sign and agree voluntarily, intending this to be a complete and unconditional release of liability for all risks of participation, including those resulting from unexpected or extraordinary conditions, to the fullest extent permitted by law.
